Zoe Primary Care offers a CDC-recognized diabetes prevention lifestyle change program to prevent type 2 diabetes and reverse prediabetes.
We take a scientifically proven approach to provide structure, education, and support to address and prevent the underlying causes of type 2 diabetes.
When you join our yearlong program, which includes both in-person and virtual sessions, you will learn, laugh, share stories, try new things, and build better habits while improving your health.

Improve Your Health.
Our lifestyle coaches are specially trained to use a CDC-approved curriculum and are experienced in supporting participants, providing guidance, and leading groups effectively.
We use the concepts of Lifestyle Medicine to empower you to lead a longer, healthier life through improved nutrition, physical activity, sleep quality, stress management, social support, and more.
We will partner with you to implement an achievable and sustainable plan that will yield lasting benefits. We alternate between in-person and virtual sessions, meeting about once a week for the first 6 months and then once or twice a month for the second 6 months.

Rebekah is a certified health coach as well as a registered nurse (RN). She helps participants uncover their values and motivators, create a wellness vision, make long-term goals that align with CDC recommendations, and break those down into attainable, short-term goals.

Am I Eligible?
To participate you’ll need to meet ALL 4 of these requirements:
- Be 18 years or older
- Have a body mass index (BMI) of 25 or higher
- If Asian American, have a BMI of 23 or higher
- Not previously diagnosed with Type 1 or Type 2 Diabetes
- Not currently pregnant
You’ll also need to meet ANY 1 of these requirements:
- Had a blood test result in the prediabetes range within the past year (includes any of these tests and results):
- Hemoglobin A1C: 5.7–6.4%
- Fasting plasma glucose: 100–125 mg/dL
- Two-hour plasma glucose (after a 75 g glucose load): 140–199 mg/dL
- Be previously diagnosed with gestational diabetes (diabetes during pregnancy)
- Received a “High Risk” result on our Prediabetes Risk Test (right)
