Intake Appointment Checklist

☐ Click the “Create Your Account” link in an email you may have received from [email protected] with the subject line “Patient Portal Account Notification” (if you did not receive this email, register for a patient portal account)

  • Text or call our office (804-303-9622) for assistance if you are unable to create/register for a patient portal account

☐ Complete all of the forms assigned to you within the patient portal OR plan to arrive 30 minutes before your appointment time to complete them in-person

☐ If the medical insurance you have is not listed on our “Insurances We Accept” page and/or you are not sure how much your visit will cost, contact your insurance company to confirm that we are in-network (our Tax Identification Number is 883506778) and inquire about the patient responsibility for a new patient office visit (CPT code: 99205)

  • The actual amount due for your first visit may be different depending on what services you receive, but this should give you a good idea of what you may be expected to pay at the end of your visit

☐ Respond to any text messages or phone calls you receive from our office (804-303-9622) to confirm your appointment

  • Requests to reschedule or cancel your appointment must be received at least 1 business day in advance. Otherwise, you may be assessed a $100 late rescheduling/cancellation fee.

☐ Arrive 15-30 minutes before your appointment time. Our address is: 4191 Innslake Drive, Suite 212 Glen Allen, VA 23060 (look for the “Humana” sign as you enter the parking lot).

  • Out of courtesy to other patients, if you are more than 10 minutes late, you will be required to reschedule your appointment. If you do not attend your appointment, you may be assessed a $100 no-show fee.
